Position Summary:
The Emergency Financial Assistance (EFA) Program is one part of the JF&CS Center for Basic Needs Assistance (CBNA). EFA provides financial assistance for low-income individuals and families who are experiencing economic challenges in meeting their basic needs. Financial assistance is provided within the context of a supportive case management relationship with the goal of assisting clients in moving from crisis toward stability.

The Intensive Case Manager is part of the EFA team, works in collaboration with other programs of the Center for Basic Needs Assistance, JF&CS programs, and CJP (Combined Jewish Philanthropies) partner organizations.

The Intensive Case Manager conducts full assessments of client needs including financial information regarding income and expenses. The case manager connects clients to all resources that they can potentially benefit from including public benefits and other community resources and provides support on an ongoing basis as clients make progress toward their goals.
Clients will include elderly, families with children, and people with physical and psychiatric disabilities. Services are strengths-based and flexible to meet the needs of each individual client situation.

Essential Job Functions:
  • Establish a trusting and supportive relationship with clients.
  • Work with the client to prioritize needs and create an action plan.
  • Connect the client with any and all JF&CS services that meet their needs, including all CBNA programs, services for people with disabilities and mental illness, senior services, and supports for new parents.
  • Coordinate needed services from CJP Partner Agencies.
  • Connect the client to community resources including the Department of Transitional Assistance, housing resources, fuel and utility assistance, and other sources of cash and cash-value support...
  • Contact service providers on the client's behalf to expedite access to needed resources.
  • Accompany the client to appointments as needed to access resources.
  • Provide hands-on assistance completing applications and gathering required documentation as needed.
  • Review the household budget to identify opportunities to reduce ongoing expenses, introduce payment plans, and avoid incurring penalties associated with late payment of bills.
  • Participate in available trainings and the CBNA Case Coordination Team meetings.
  • Facilitate the allocation of financial assistance grants on behalf of clients.
  • Make home visits or meet with client in the community as needed.
  • Follows all JF&CS client record and documentation protocols.
  • Utilizes the JF&CS client data system, ETO (Efforts to Outcomes)
  • Other duties as assigned 

Educational/Experience Requirements:
  • Bachelor's degree is required. Master's degree in social work is preferred.
  • Two years of full-time or equivalent part-time, professional experience in case management i.e. working with public benefit systems, coordinating services, working with low-income populations.
  • Cultural competency and ability to establish effective professional relationships with diverse groups of clients.
  • Strong verbal and written communication skills.
  • Strong organizational skills and high level of attention to detail.
  • Knowledge of Microsoft Office Suite (excel, word, outlook, power point)
