Peachford Hospital, a leader in behavioral health and addictive disease treatment, based in Atlanta, Georgia,  is searching for master’s prepared Case Managers to be a part of our Social Services team.

What Role Will You Play?
Social Services Case Managers at Peachford are part of an interdisciplinary treatment team charged with providing quality therapeutic and case management services to our patients from admissions to discharge.

What Will You Get To Do?
Social Services Case Managers are responsible for completing the following duties and tasks:
  • Conduct psychosocial assessments to document patient history and identify preliminary issues for treatment focus by interviewing the patient and his/her family members.
  • Provide individual, group and family solutions-focused interventions using appropriate therapeutic techniques and models. Facilitate process-oriented and didactic groups.
  • Document treatment and case management provided to patient in the patient’s electronic medical record.
  • Develop and coordinate an individualized discharge plan for the patient by utilizing information obtained from the patient, treatment team and medical record to determine discharge planning needs. Identify available appropriate family or community resources such as group homes, halfway houses or treatment providers.
 
What Does Peachford Require for this Role?
A license eligible master’s degree in a related clinical area is required as well as the desire to help our patient’s improve their lives. A positive attitude and outlook will make you successful in this role.
